The Clinical Care & Practice Operations Manager will manage the multidisciplinary clinical team for HammondCare Home Care across Central Coast, Hunter, Northern NSW and QLD.
This is a Permanent Full-time position based on any our HammondCare locations across the region as well as the ability to work from home.

Lead and strengthen interdisciplinary clinical practice to ensure safe, high-quality, evidence-based care that aligns with client goals, funding requirements and HammondCare standards.
Use data, systems and coaching to monitor and improve performance, enhance reliability, reduce variation and achieve service-level KPIs across all funding streams.
Drive service design, innovation and efficiency—partnering with key stakeholders to build sustainable care models, optimise workforce skill mix and support scalable growth.
Build and develop a capable, engaged and high-performing workforce through coaching, clear expectations, effective workforce planning and fostering a positive, collaborative and psychologically safe culture.
Provide leadership in risk management and compliance, ensuring strong governance of clinical risks and incidents, adherence to regulatory requirements, and robust systems that support safe, reliable and continuously improving care.
Registered Nurse with a current AHPRA registration
Significant operational and managerial experience leading clinical or interdisciplinary teams in complex care environments.
Strong performance management capability, including running service rhythms (dashboards, huddles, reviews) and achieving service-level KPIs.
High-level data literacy, with the ability to interpret incidents, audits, outcomes and workforce metrics, and translate insights into operational action; confident using BI tools (e.g. Power BI).
Advanced leadership and coaching skills, including setting clear expectations, building capability and managing underperformance effectively. In-depth understanding of aged care funding and policy, including multi-funding environments (e.g., Support at Home, CHSP) and relevant manuals, standards and compliance requirements.
Demonstrated ability to operate in fast-paced, high-detail environments, managing multiple priorities while ensuring accuracy, reliability and regulatory compliance.
Strong communication and stakeholder engagement skills, with the ability to work across multiple sites and collaborate effectively with staff, clients, families and external partners.
Willingness to travel between sites and hold a current Australian licence and access to their own car
